Your assessment details

AZ Kosher Tours uses the contact, travel, group, kosher, dietary and accommodation details you provide to respond to your trip request and prepare a proposal. Please include only details needed for planning. Do not enter payment information or identity documents.

Selected services, quote requests, invoices, verified payment records and private follow-up notes are kept with your client record. Zelle payments are verified manually. Card details should never be entered in this form. Submitted assessments are stored on AZKT’s hosting account and emailed to the business owner. Your email address is used for a confirmation and trip follow-up. Information needed to arrange services may be shared with relevant providers after the arrangements are discussed with you.

An unfinished form is temporarily kept in this browser tab for up to 24 hours. Downloaded recovery copies remain on your device until you delete them. A recovery copy is not a submission.

Optional marketing consent is separate from your trip request. To request access, correction or deletion, email yaakovtheyid@gmail.com. AZKT retains submissions while needed for the request and its business records; review and delete outdated records periodically.
